Chiropractic care represents a safe and effective treatment alternative for older adults, according to evidence-based recommendations published in the May issue of the Journal of Manipulative and Physiological Therapeutics

“The most important take-home point from this updated document is that given the growing need for evidence-based, cost-efficient care in the older adult population, chiropractic care is a very attractive alternative,” explained lead author Cheryl Hawk, DC, PhD, Professor at Texas Chiropractic College in Pasadena, Texas.

“Chiropractic care incorporates many different treatment modalities, as outlined in the document,” Dr. Hawk told SpineUniverse. “One of the key treatments that chiropractors deliver is spinal manipulative therapy (SMT). The evidence base for SMT continues to grow.”

Spinal Manipulation Is First-Line Treatment for Low Back Pain

“The most important recent update is the new American College of Physicians clinical practice guideline, in which spinal manipulative therapy (SMT) is included as a first-line treatment for low back pain,”1 Dr. Hawk said. “In addition to SMT, these updated clinical practice guidelines also recommend other treatments commonly delivered by chiropractors, including exercise recommendations.”

The Best Practices paper lists considerations to apply when treating older adults with manual procedures (Table 1). In addition, the recommendations emphasize the need to incorporate the biopsychosocial model of pain in the assessment and management of older adults, including age-related changes in the neurologic and musculoskeletal systems of older adults that may require consideration during physical examination (eg, peripheral neuropathy, muscle loss, and decreased flexibility).
Considerations for Treating Older Adults With Manual Procedures.“Health professionals caring for older adults must recognize that these patients may have cognitive issues, depression, substance use issues, or other factors that make their care complex,” Dr. Hawk said. “It is essential that providers caring for geriatric patients become fluent in evidence-based screening tools to assure that the entire clinical picture is being considered.”

How to Counsel Elderly Patients on the Safety of Chiropractic Care

The updated recommendations include data from recently published randomized-controlled trials that provide additional support for the safety of SMT in older adults.

“The most valid studies suggest that about half of all patients will experience adverse events after SMT,” Dr. Hawk explained, adding that the events are usually mild (muscle soreness or stiffness) and transient (24-72 hours). “No reliable data exist about the incidence of serious adverse events, but they appear to be extremely rare.”

“Based on our review of the literature on safety, reported in our paper, the bottom line is that the risk for cerebrovascular accidents associated with SMT in the older adult is minimal given that even the small risk that exists is in a younger (<45 years of age) population,” Dr. Hawk said. “It is important for ALL health professionals to be aware of and to screen patients for risk of cervical arterial dissection and inform patients of their risk.”2

“Similarly, ALL health professions should recognize stroke risk factors and counsel patients on prevention strategies, and risks,” Dr. Hawk said. “It is noteworthy, that the risk of stroke after a medical visit is slightly higher than the risk of stroke after a chiropractic visit. In both cases, patients who have bad headaches seek care and all health professionals should be vigilant for signs of stroke and make appropriate referrals. Chiropractic care has been shown to be safe in the general population.”

Chiropractic Care May Be Least Invasive Option for Musculoskeletal Pain

“Both chiropractors and other professionals try to care for older adults using a ‘least invasive to most invasive’ approach,” Dr. Hawk concluded. “Often, complementary and alternative therapies, such as chiropractic (SMT) may be that least invasive option.”

“We all strive to care for all of our older patients as if they were our own grandmothers,” Dr. Hawk added. “Particularly in the management of musculoskeletal pain, chiropractic care may be the most safe, effective, and cost effective conservative management strategy.”

As practicing chiropractors, we still face much misunderstanding and bias against our care from many medical specialists. The most pervasive myth we face is that chiropractic is risky for certain populations, particularly the elderly. I think this review article covers that topic in detail, and makes a strong case for the safety and effectiveness of this option.

It can be challenging to council elderly patients about the safety of chiropractic care, especially since many patients are misinformed about the topic. As this paper notes, most doctors of chiropractic are well aware of contraindications to care and “red flags” suggesting a patient requires a referral for other specialty care.  It is also important to educate elderly patients that all treatments will be customized to a particular patient’s needs and abilities. Many patients (and medical specialists) associate chiropractic treatments only with high velocity manipulation. While this approach is safe and effective for most patients, chiropractors are able to modify their treatment approach based on a patient’s medical condition or personal preference.

 One important point that is not well discussed in this article is that many of the so-called “conventional” approaches to spinal pain carry significant and often under-appreciated safety concerns, particularly in the older population. For example, non-steroidal anti-inflammatory drugs (NSAIDs) are often a first-line treatment for spinal pain, even though there is little evidence for their effectiveness. These drugs carry significant risks of gastric ulcerations, especially among elderly patients. Recent studies have also demonstrated these drugs have cardiovascular risks, such as increased risk of heart attack and strokes. Skeletal muscle relaxants are also often used to treat spinal pain, but they have risks that are concerning among elderly patients, including behavioral problems and increased risk of falls.  Finally, narcotic pain relievers, despite all their well-recognized risks of addiction and abuse, are commonly prescribed to patients with back pain. One study found 45% of patients with back pain took a narcotic, but found that patients were less likely to take a narcotic after receiving chiropractic services.

Even though the biopsychosocial model of spine pain is a relatively new concept, I think aspects of it have long been a part of chiropractors’ approach.  Our non-pharmaceutical emphasis has always included encouraging the patient to return to normal activity and to take an active role in recovery. Also, the unique relationship many patients have with their chiropractors is often based on relatively frequent visits where the patient receives not only active treatments like manipulation and exercise, but also gets positive encouragement and useful advice.

I hope that this study will encourage your readers to take a fresh look at chiropractic care for their older patients, and perhaps inspire them to seek out an evidence-based doctor of chiropractic to help them safely and effectively co-manage elderly patients with spine pain.”

Un estudio realizado en Nueva York, comparo el uso de analgesicos y la terapia de quiropráctica en 200 pacientes. Luego de 6 meses, se encontro que los dolores de cabeza de las personas que se habian sometidoa la manipulación manual notaron una reducción de los dolores de cabeza que padecian y a diferencia de los medicamentos, se trata de una solución permanente que puede conservarse en el largo plazo con las indicaciones adecuadas.

Técnicas de Ajuste Quiropráctico

Las dos técnicas más comunes son las denominadas manipulación y movilización . En la manipulación de alta velocidad y baja amplitud se utilizan las manos de manera vigorosa para devolver a la articulación su movilidad y eliminar cualquier adhesión que exista aumentando el riego sanguíneo hacia los órganos. Según el caso y las preferencias de cada paciente, se puede utilizar una técnica más suave, que recibe el nombre de movilización espinal. Otro método es conocido como flexión distracción se realiza haciendo uso de una camilla especial donde se realiza un bombeo discal ayudándose de la fuerza de gravedad que se genera con la postura del paciente en la camilla. Are Chiropractic Adjustments a Temporary Fix?

The chiropractic profession is filled with a diverse group of doctors all in search of the answer to long term health through removal of subluxation. The philosophy that is taught in chiropractic colleges across the country is sound in theory, but lacks the clinical protocol to back it up. Subluxation a so-called bone out of place theory is out dated and unrealistic. Individual bones or segments of the spine do not mis-align unless the ligaments are torn, a rare condition caused by significant trauma or genetic connective tissue disease. The majority of spinal subluxation, a word that should be redefined or gotten rid of, implies that there exists a bone out of place when in fact every radiology report that I have ever received from a radiologist states “no subluxation is present”. But yet you go to the chiropractor and he or she says;”yep there it is it’s your C5, T7, and L4 they are out of place and need to be adjusted back into place” Then you receive the adjustment “Crack,Click,Drop” there you go all set, back in place. Right? Wrong.

I have been in practice for over 10 years now and about 60% of the patients I have examined have had chiropractic adjustments before. How many of these patients had normal posture and healthy alignment on x-ray? None. Why? Because chiropractic adjustments do not correct bad alignment. If we look at the research regarding spinal biomechanics we understand that the spine and lower extremities are linked together and that your body misaligns globally, also why a radiologist won’t pick it up. In addition there has to be an origin or primary event which starts this chain reaction. The spine and extremities compensate after the primary event occurs, creating a global postural distortion. So when you show up in the chiropractors office, he or she will use segmental analysis to find the hot spots(subluxations) and adjust those segments, temporarily changing the natural compensatory mechanism that was created by the initiating event. In english this means you get a temporary shift in your pattern, which often, about 80% of the time, creates a good feeling or a sense of wellness. Once in a great while the initiating event is so fresh, that the one adjustment actually corrects the global alignment, very rare though less than 1% of the time.

So what is an adjustment doing? A spinal adjustment or sudden applied force to a spinal segment causes joint movement slightly beyond the physiological barrier of passive motion. Huh! It unlocks restricted joints in your spine or extremities. Because it releases the restricted joint, it has a temporary relaxing effect on the muscle that moves that specific joint. This causes a temporary increase in range of motion, decreases pain through the thalamic(section of the brain) system, and decreases muscular tension. Oh by the way, these are all good things! The problem isn’t with the adjustment, the problem is that your postural mechanism will cause you to return to your pre-adjustment position within hours of the treatment. So in reality you feel better for a while, but your initiating event wasn’t corrected so the poor alignment returns and so does the tension, stiffness, and pain. The nervous system also remains unhealthy do to the postural distortion that is left uncorrected. Your body shortly after the adjustment coils itself back into it’s unhealthy position.

So what about exercise or stretching alone or in combination with the adjustment? Close but no cigar. Active exercise is great for a body that has good alignment and proper mechanics, but not so good for someone with bad alignment/posture and poor body mechanics. Active exercises like core strengthening and isometrics do not affect the slow twitch muscles that control posture. Stretching feels great but is temporary. Also if you stretch a muscle that is programmed to be shortened by the brain to help stabilize the spine it can cause further problems. You can now see that simplifying a very complex system to include adjustments and exercises just won’t cut it. This probably explains why we still have 80% of the population suffering with back and neck pain but yet chiropractic has been around for over 100 years and who knows how long exercise has been prescribed for back problems.

The majority of people with chronic problems have poor postural alignment. This poor alignment stresses certain joints which eventually cause pain and even arthritis if not corrected. Poor alignment causes certain muscles to be overworked which leads to myalgia (muscle pain) due to a lack of blood flow through the overly tense muscle. This decrease in blood flow causes excess metabolic waste accumulation in the muscles and eventually can cause permanent fibrosis commonly referred to as fibromyalgia. These babies get rock hard and sensitive! The other often overlooked component of poor alignment are ligaments. Ligaments connect the bones to create stability and are non-contractile tissues, meaning they can not be exercised back to health. Ligaments are also the primary reason you can not change alignment through exercise and chiropractic adjustments. This must explain why 95% of chiropractors do not take post x-rays, they know the spinal alignment on x-ray will not have corrected.

It is published in many prestigious spine journals, anatomy text books, and other related spinal health literature that your spine and posture have measurable healthy normals. The best and most reliable way to check the health of a persons spinal structure is with x-rays and posture analysis (Dr. Ventura’s Posture Analysis software is very accurate). Now if I can measure your poor alignment with an x-ray and posture analysis, would it not make sense to take another x-ray and do posture analysis after the treatment to see if I did my job?

The good news is that through many research studies and new technology we have found a good, consistent way to correct poor alignment. Spinal adjustments combined with re-active muscle training and sustained ligament traction are very effective for long term correction. In addition, traction force chiropractic adjustments are far more effective at breaking up adhesions in chronically restricted joints than the traditional compressive adjustments taught in chiropractic college, including instrument and drop table adjusting. Re-active muscle training makes sense because your posture is merely an adaptation to the environment that you put it in. for example if you slump at your desk and stare at a computer all day, your body will literally mold into your slumped forward position. If you always use your right hand to carry stuff, the natural compensation for this will be overuse of the left side spinal muscle to help stabilize the added weight to the right side of your body. Reactive muscle training uses the principals of compensation. If you have a right dominant posture (you lean right while standing straight) measured with posture analysis, I would add weight to your right side to cause an automatic brain controlled activation of the opposite side muscles causing your body innately to move its posture to the left. Bingo, we are starting to get somewhere. In addition to adding weight, I would have you stand on a vibration platform set at a specific frequency and amplitude so the postural muscle activity would be increased by about 30 times resting levels. This speeds the learning process up so you don’t need 5 years to get it done. The shortened ligaments only, need to be elongated. This is done trough pressure(stretch) over time. A ligament typically will respond after about 15 minutes of pressure. This uncoiling process is termed “ligament creep”. What is the severity of the condition and how long has it been present?

First let’s get the insurance question out of the way. When it comes to chiropractic treatments, often the only kinds of insurance policies that will pay for it (including at home massage) are auto insurance or workman’s compensation insurance for car and work injuries. Massage therapists can not officially diagnose any condition, so a doctor’s prescription is always required. This assumes that the individual would be open to using medication, because in addition to the myriad of tools at physician’s disposal, it is highly likely that a doctor will prescribe an anti-inflammatory or possibly a prescription pain killer if the pain is severe.

It’s important for people to understand that while massage therapy can be very effective in reducing pain caused by muscle tension and related restrictions, it can only achieve so much with one visit. Typically, in order to address more chronic kinds of pain, multiple sessions will be required to make significant headway. Therefore, it is often not a bad idea to see a doctor who may be able to prescribe medication to help the condition, both in terms of reducing pain immediately, and also for reducing inflammation in general, which is often largely responsible for musculoskeletal pain. These efforts can actually make subsequent massage therapy sessions even more effective.

Think of the tight muscles like a frozen stick of butter. If you try to press into it when it’s right out of the freezer, it’s almost impossible to make a dent. However if it has been defrosted to room temperature (like muscles on an anti-inflammatory), it could be in a much easier state to work with in the care of able hands. This being said, it’s also important to be careful about the use of pain killers before getting a massage and advise the therapist of the current use of any medications.

Another option for those with muscle pain which would fall somewhere in between going to a medical doctor and seeing a massage therapist would be going to see a chiropractor. While a chiropractor can not prescribe medication, they can diagnose certain conditions, perform x-rays, and provide natural therapy including “adjustments” among some other things that massage therapists are not authorized to do. In some cases they may also be able to accept normal major medical health insurance along with a co-payment. For some people this is a good “in-between” option providing a relatively high level of care that is also “natural”. At the same time, there can be drawbacks.
